Recognizing the benefits of remote patient monitoring to patients and the wider healthcare system, Medicare has created significant opportunities to generate new revenue.
99453 - Patient education and setupInitial patient education and setup. This is billed once per episode of care, which must be a minimum of 16 days in length.
99454 - Kit supplySupply of the remote patient monitoring kit. This must be capable of at least daily transmission and alerts. This is billed once per 30 days. The minimum length of the episode of care is 16 days.
99457 - Initial 20 minutes of monitoring timeThe first 20 minutes of monitoring time within a calendar month. This is billed once per 30 days. The service is delivered under general supervision, meaning that it does not need to be the physician who does the monitoring.
99458 - Each additional 20 minutes of monitoring timeEach additional 20 minutes of monitoring time within a calendar month. There is no limit to the number of times this can be billed in a calendar month. The service is delivered under general supervision, meaning that it does not need to be the physician who does the monitoring.
G2012 - Virtual check-inThis is for a brief technology-based communication, e.g. video visit, by a physician or other qualified health care professional who can report evaluation and management (E/M) services, provided to an established patient, not originating from a related E/M service provided within the previous 7 days nor leading to an E/M service or procedure within the next 24 hours or soonest available appointment; 5-10 minutes of medical discussion.
